Kidney Transplant Outreach Coordinator – Kidney Acquisition – Full Time 8 Hour Days (Exempt)(Non-Union) – (Los Angeles, California, United States)

The Kidney Transplant Outreach Coordinator works in collaboration with dialysis centers and referring providers to generate and support kidney transplant referrals. Liaison between the kidney transplant team and dialysis centers and referring providers to build and maintain positive relations, increase referrals, provide education and information between the groups, and to mitigate and/or resolve issues, problems and challenges. The Kidney Transplant Outreach Coordinator serves as a resource to dialysis centers, their social workers and referring partners and other professionals in the kidney transplant arena and will be expected to be in field three to four days a week meeting with dialysis centers and referring providers providing education, patient updates, reports, and other information promoting kidney transplant and kidney living donation. Monthly reports and summaries will be generated and sent to Transplant Leadership on the productivity of the outreach. The Kidney Transplant Outreach Coordinator will collaborate and work with Transplant Leadership and the Kidney Transplant Staff to ensure information is communicated appropriately. The Coordinator will update and maintain kidney transplant education materials, marketing materials, and event information to current times and will be responsible for providing these materials to dialysis centers and referring partners. Takes lead in ensuring the USC Kidney Transplant Program participates in community events, table/booth set-up at dialysis centers and applicable conferences. May be required to attend continuing education conferences, workshops, and in services to maintain up to date knowledge and awareness regarding pertinent topics in Kidney Transplant and Kidney Living Donation

Essential Duties:
• Community Outreach: Visits dialysis centers and referring partners three to four days a week to build and maintain positive relations, increase referrals, provide education and USC kidney transplant and living donation information, and to mitigate and/or resolve issues, problems and challenges. Maintains point of contact for the dialysis centers and referring partners and ensures availability of their time. Works independently and ensures reliability of work provided to our partners. Provides quarterly or bi-annually dialysis communication reports to all dialysis centers within the community and surrounding counties as applicable. Identifies community events and proactively participates in community events, table / booth set up, and presentations.
• Education and Training Development: Provides support, education, and advocacy to patients and potential donors. Proactively liaises our medical/surgical directors with the referring by arranging for on site visits for the community nephrologists at Keck, arranging for CMS education seminars (virtual or visits) for dialysis units (MDs, SWs, RNs), provides the multidisciplinary team (NPs, RNs, LCSWs, RDs) opportunities to visit the DUs for the purposes of outreach. Actively promotes living donation through microsites, NKR and works in collaboration with living donor team to educate and expand living donation at the dialysis units Develops kidney transplant and living kidney donation education materials (e.g. presentations, brochures, pamphlets, etc.) in collaboration with transplant leadership and provides the education to the dialysis centers and referring partners.
• Collaboration with Transplant Team: Manages challenges/conflict and escalates to transplant leadership when necessary. Keeps transplant leadership informed of this type of communication and what has been done to mitigate issues. Collaborates with the kidney transplant team and living kidney donation team to improve work processes, identify and address barriers that may impact referrals, and works to maintain good communication between the USC Kidney Transplant Department and dialysis centers / referring partners. Provides support to the USC Kidney Transplant Program wherever and as needed
• Documentation: Provides support in the screening of re-referrals to rule out any contraindications to transplant (e.g. psychosocial contraindications) Provides support and collaboration with the intake team to ensure effectiveness and efficiency of referrals and the referral process. Will assist with gathering missing patient information and uploading patient documentation to the transplant database and/or electronic medical record. Works collaboratively with the Pre-Kidney Transplant Team throughout the pre-phases of transplant (referral, evaluation, and listing). Responsible and accountable for following the USC KNOWN standards, maintaining a positive attitude, advocating for USC Kidney Transplant and Living Kidney Donation and promoting the USC Kidney Transplant Program.
• Performs other duties as assigned and as needed.

Minimum Education:
• High school or equivalent

Minimum Experience/Knowledge:
• Req Bachelor’s degree Degree in a Healthcare related field required
• Pref Master’s degree Degree in Business Marketing or in Healthcare related field, strongly preferred
• Req 3 years Minimum of three years of experience in dialysis, or kidney transplant and kidney living donation
• Req 3 years Minimum of three years outreach and marketing experience.
• Req Provides a patient-oriented attitude with strong advocacy skills for transplant and donation.
• Req Has strong negotiation skills, ability to influence, is reliable, dependable and independent needing little support or supervision.
• Req Adapts easily to change, can manage challenges / conflict and ensures strong professional oral and written communication.
• Req Excellent in building relationships and rapport.

Required License/Certification:
• Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

Salary Range:
The annual base salary range for this position is $68,640.00- $112,370.00.
